Care Finder Program

• People that have no support network or family that can help them.

• Must not have a carer or support person they can trust or feel comfortable with.

• Be eligible for government funded aged care.

• Have communication barriers language and literacy.

• Difficulty understanding information and making informed decisions.

• Be hesitant to engage with aged care or government services.

• Be in a vulnerable or unsafe situation if they don’t receive services.

If someone needs Care Finder support a local organisation will contact the person and organise a meeting appointment, usually in person and somewhere the person feels comfortable like their home or somewhere safe they choose. The care finder will ask the person questions to get a holistic view of their situation and help the person work through to meet their needs.

• Talking to MY AGED CARE on their behalf and arrange an assessment.
• Attending and providing support at the assessment
• Help with finding suitable aged care providers in the area.
• Help fill out forms and understand aged care service agreements.
• Checking in service to see how things are running making sure services are still              appropriate.
• Solving other challenges connecting to supports in the community such as health, mental health, housing and homelessness, drug and alcohol services and community groups.
